Failure Modes and Effects Analysis (FMEA)

Failure modes and effects analysis is a structured team tool that ranks causes of failures using a summary measure of their importance and likelihood of occurrence. A team brainstorms “types” of failures, or failure modes. Root causes of these failure modes, and their effects, are then determined. For each combination of root cause and its effect, the team assigns ratings that reflect the severity of the effect, the likelihood of occurrence of the root cause, and the ability to detect the occurrence of the root cause. These values are multiplied together to give a Risk Priority Number (RPN). Large RPNs reflect areas that require attention, and corrective actions should be expended to reduce the RPNs for the associated causes and effects to an acceptable level of risk.
